1. Create a form with the following fields:

• Name (text field) -> check name submission / Advanced tab: mandatory field
• E-mail (text field) -> check e-mail transmission / Advanced tab: mandatory field
• Request (Text field multiline)
• Submit (submit field)

2. now integrate the form using a powermail plug-in.

3. test the form by filling it out and see what happens.
